Application for Withdrawal of Pension Contributions
Use the Application for Transfer/Withdrawal of Pension Funds form if you have terminated from the Pension Plan and are eligible to withdraw your pension funds from your Plan.
You can only withdraw your pension funds if you are ‘eligible’. Please check with the Plan Administrator to determine if you are eligible for withdrawal.
Information Needed to Complete the Form
The Member’s personal information is needed to complete the form. In addition, you must select how you wish to receive your pension funds that are to be paid out.
Additional forms are required in addition to the Application for Withdrawal of Pension Contributions, if:
- Your pension funds are ‘locked-in’, you must also complete the: Royal Trust Locked-In Funds form.
- Your pension funds are not ‘locked-in’, you must also complete the: Direct Transfer of a Single Amount (T2151) form.
Please check with the Plan Administrator if you are unsure of whether your funds are ‘locked-in’ or not.
Completed forms should be forwarded to the Plan Administrator.
