Employer Remittance Reports

Remittance reports allow the employer to calculate and report the hours an employee worked and the corresponding remittance due to the Heat and Frost trust fund.

Remittance reports are available for both the: Commercial and Institutional Plan, and Industrial Plan.

Remittance Report – Commercial and Institutional Plan

Use the Commercial Remittance Report to calculate and remit the amount due to the Health & Welfare and Pension Plans for regular members of the Commercial Plan.

Use the Commercial – Associate Member Remittance Report to remit amounts due for associate members of the plan.

Information Needed to Complete the Form
In addition to the employer information, the following information is needed to complete this form:

  • Employer information
  • Employee information,
  • Hours detail, and
  • Calculations to determine the total amount remittable by the employer.

Remittance Report – Industrial Plan

Use the Industrial Remittance Report to calculate and remit the amount due to the Health & Welfare and Pension Plans for regular members of the Commercial Plan.

Use the Industrial – Associate Member Remittance Report to remit amounts due for associate members of the plan.

In addition to the employer information, the following information is needed to complete this form:

  • Employer information,
  • Employee information,
  • Hours details, and
  • Calculations to determine the total amount remittable by the employer.

Please contact the Plan Administrator for a copy of your remittance report. Detailed instructions on completing the forms are found on the forms. Questions on completing the form should be directed to the Plan Administrator. Completed forms should be sent to the Plan Administrator.
