Request by Limited Member for Transfer or Pension (Form 4)

Use
Once the spouse has been registered as a "Limited Member" using the
Request for Designation as Limited Member of Pension Plan (Form
2), the spouse
may now select how he/she wishes to divide the Pension using the
Request by Limited Member for Transfer or Pension (Form 4). This form is used by
the Member’s former spouse after a marriage breakdown (divorce).

Information Needed to Complete the Form
To complete the form, the spouse must include:
- Plan information,
- Limited Member's personal information,
- Plan Member personal information, and
- information related to the Limited Member's request.

Notes
This form is used for an unmatured Pension (the Member has not yet retired) in
a Defined Benefit plan. The spouse may select to transfer his/her share of the
Pension to a separate account (if the Member is eligible to retire) or
request a separate Pension, payable directly to the spouse, when the Member
retires.
If the agreement or court order between the spouse and the Member was made prior to
July 1, 1995, the spouse should complete the Request in Relation to a Matured Pension
Divided under an Agreement or Court Order Made Before July 1, 1995 (Form 5) instead of
this form.

Administrator.
Completed forms should be forwarded to the Plan
Administrator. |
