Information Needed to Complete the Form
In addition to the employer information, the following information is needed to complete this form:
- Employer information
- Employee information,
- Hours detail, and
- Calculations to determine the total amount remittable by the employer.

Notes
Rates for each employee class can be found on the form.
Questions on completing the form should be directed to the Plan Administrator.
Completed forms should be sent to the Plan Administrator with copies to other organizations as detailed on page 2 of the form (item #6).
